This 1:1 customized program is just for you! Together we will explore your treatment plan from your chosen practitioner along with your wellness goals. This in turn, will get the most our of your treatment plan as well as help yourself and your family achieve a healthier diet and lifestyle. I take an individualized approach to teaching you how to make it all work in your life. You will learn lifelong skills to not only improve your health and wellness, but to be able to improve your family’s. You will learn how to improve your nutrition, but also lifestyle practices that are proven to help you handle stress better, sleep better, and get back to enjoying life!

What’s Included:

Health History Intake - In order to meet your goals, we have to know where you currently are in regards to nutrition, mood, sleep, and lifestyle. This is an in-depth form that takes a deep dive into your life. You will explain your food habits, cooking ability, and lifestyle habits. You will need to share your Health Care Plan from your practitioner.

Initial Consultations - Two 60-90 minutes consultations allow a deep dive into your health and wellness goals. Once the intake form is completed and your practitioner’s Treatment Plan is received, the first initial consultation meeting will be scheduled to review the Intake Form and Treatment Plan. The second consultation meeting will be held up to a week after the first to review your individualized program in detail.

Foods to Avoid/Enjoy List - In conjunction with your practitioner’s nutritional recommendations, the Program includes Food to Avoid/Enjoy List.

Recipes - 4 Breakfast, 6 lunch, and 6 dinner (entree + side) recipes that align with your treatment plan.

Food Swaps Guide - Guide for healthier versions of your favorite items. There’s a healthier alternative to everything. No need to feel deprived!

4 Coaching Sessions - Once we review your individualized program, every two to three weeks, we will celebrate your successes as well as address struggles and challenges. This is the core of the program where you learn tools through a customized learning plan for what you need to know about nutrition, movement, sleep, stress management, and lifestyle changes. These all important life skills will create new habits to practice for the rest of your life.

Re-Assessment - Check-ins along the way to measure your growth as well as address struggles and set backs. Healing is never linear!
